What Is a Machine Safety Priority Matrix?

A Machine Safety Priority Matrix is a decision-support tool used to help organize machine safety findings into relative priority levels.

It bridges the gap between identifying a potential concern and developing a risk-reduction plan.

The matrix helps teams move from:

“We have a list of machine safety issues.”

to:

“These are the areas we believe deserve attention first.”

It is not intended to establish an ANSI risk level, certify that a machine is safe, or replace a formal machine risk assessment. Instead, it provides a practical framework for organizing findings and supporting better machine safety planning.

Why Prioritization Matters

A facility may identify dozens—or even hundreds—of potential machine safety improvements.

Addressing everything simultaneously is rarely practical.

Budget limitations, production schedules, engineering resources, equipment availability, shutdown windows, and project complexity can all affect when improvements can be implemented.

A priority matrix helps bring structure to those decisions.

Instead of relying solely on which problem was discovered first, which department raises the greatest concern, or which improvement is easiest to complete, teams can use a consistent framework to determine where additional attention should be focused.

What Should Be Considered?

The Machine Safety Priority Matrix can help teams consider several factors when reviewing an identified concern:

Potential Severity

What could reasonably happen if someone were exposed to the hazard?

Consider the potential consequence of an incident rather than simply whether an incident has occurred previously.

Exposure

How frequently do operators, maintenance personnel, setup technicians, or other employees interact with or approach the hazard?

Routine interaction may warrant different consideration than infrequent access.

Existing Safeguards

What risk-reduction measures are already in place?

Consider physical guarding, interlocks, presence-sensing devices, safe operating procedures, control measures, and other existing protections.

Personnel Interaction

How do people actually interact with the machine?

Operation, loading, unloading, setup, adjustment, cleaning, troubleshooting, maintenance, and recovery from jams may create very different exposure conditions.

Operational Considerations

Are there practical conditions that could affect implementation?

Production schedules, planned shutdowns, equipment replacement, engineering complexity, available resources, and other factors may influence the corrective-action plan.

From Findings to Priorities

The purpose of the matrix is to create a more structured conversation around machine safety priorities.

A simple process might look like:

IDENTIFY → REVIEW → PRIORITIZE → PLAN → REDUCE RISK

1. Identify

Document potential machine safety gaps using the Machine Guarding Gap Checklist, an assessment, inspection, or another internal review process.

2. Review

Consider the hazard, potential severity, exposure, existing safeguards, and how personnel interact with the equipment.

3. Prioritize

Use the matrix to group findings into relative priority categories and identify conditions that may warrant more immediate evaluation.

4. Plan

Determine what additional assessment, engineering, budgeting, scheduling, or risk-reduction activity may be required.

5. Reduce Risk

Develop and implement appropriate risk-reduction measures based on a qualified evaluation of the machine and its hazards.

A Priority Is Not the Same as a Risk Rating

It is important to distinguish project priority from a formal machine risk assessment.

A priority matrix can help determine where an organization should focus its attention, but it does not replace the risk-estimation and risk-reduction process used in applicable machine safety standards.

For example, two findings could ultimately require similar risk reduction while having different implementation priorities because of exposure, production schedules, planned equipment replacement, or other operational considerations.

The matrix helps organize the work. A risk assessment helps evaluate the risk.
